Transforming patients into partners

Supported self-management platform
bridging services and citizens.

We’re on a mission to empower people to be active partners in their health and enable clinical teams to be more effective. View case studies below or contact us to find out how this platform can connect your services and citizens.

Weight Management (NHS Lanarkshire)

We are working with NHS Lanarkshire to digitise weight management services, replacing manual processes with a secure digital ecosystem. The platform connects citizens, clinicians, and community coaches, providing a streamlined approach to health tracking and data-driven support to improve long-term patient outcomes and service efficiency.

  • Empower citizens to manage their health via interactive goal setting and progress tracking tools.

  • Provide clinicians with real-time data insights to deliver more proactive and personalised patient care.

  • Automate administrative tasks to reclaim clinical capacity and improve overall healthcare service delivery.

Supporting self-management with CHSS

In partnership with Chest Heart & Stroke Scotland, Daysix is developing digital solutions to support individuals living with chest, heart, and stroke conditions. We focus on bridging the gap between clinical care and home life, providing users with the resources and confidence needed to manage their recovery and long-term health effectively.

  • Deliver a digital platform for citizens to track symptoms and access tailored recovery resources.

  • Build a clinician dashboard for monitoring patient progress and identifying those requiring urgent intervention.

  • Create integrated communication tools to streamline the transition from hospital to community-based support.

Supporting health in the workplace

Through the CivTech programme, Daysix is developing a digital managed pathway to support individuals with musculoskeletal and other long-term conditions. This project focuses on providing hyper-personalised guidance to help people self-manage their health effectively while maintaining or returning to their careers with confidence.

  • Deliver a hyper-personalised digital pathway tailored to an individual’s specific health and workplace needs.
  • Provide interactive self-management tools that build user confidence in managing symptoms during work hours.
  • Enable seamless coordination between individuals, employers, and healthcare professionals to plan safe return-to-work scenarios.

Advancing provision for MS management

Daysix is collaborating with the Precision MS team to develop digital tools that support a more personalised approach to managing Multiple Sclerosis. By offering tailored experiences, the platform aims to help clinicians and patients navigate post diagnosis and tailor support and gudiance to the specific needs of the individual.

  • Develop a secure platform to act as a bridge between clinician and citizen

  • Create intuitive interfaces that help clinicians and patients make informed, collaborative treatment decisions.

  • Improve long-term health outcomes by identifying the most effective interventions for each unique case.
